• Problem: Authorization form is missing a signature
  • Solution: Authorization form must be manually signed using a blue or black pen
• Problem: Authorization form is missing the company name that ordered the report (on page 1)
  • Solution: Authorization form must contain the hiring company name
• Problem: The checkbox on page 3 is checked and returned to GoodHire
  • Solution: The checkbox on page 3 should not be checked (unless you are not providing consent for the check)
• Problem: The address in country on the information form is not an address outside of the United States
  • Solution: Ensure the address on the information form is the last (or current) address you resided in for that country
• Problem: Full name is not provided in any of the name fields on any forms
  • Solution: Full name must match across all forms
