Provide the direct contact information for your former boss or other colleague currently at the company. We’ll reach out to them and try to get connected to the HR department. If you do not have any of this information, please provide any contact information you may be able to locate online.

How can I change the Contact After Date after I’ve submitted my check?

To change the date for contacting your current employer, please sent a written request to [email protected]. Please include your name and the employer and the date that you want to change it to. Once received, we will send you a confirmation that your request was updated.
