Keep In Mind
The Abstract of Driving Record Release of Interest requires the signature of the Employee/ Prospective Employee/Volunteer, as well as the signature of an Authorized Representative of the Employer/ Prospective Employer/ Volunteer Organization. You are responsible for completing the form, collecting the appropriate signatures, and submitting the electronic copy of the completed form to GoodHire.

STEP 1 – Authorized Company Representative must complete the following fields:
- 1A: Company name
- 1B: Company Address
- 1C: Authorized Representative Name
- 1D: Title
- 1E: Date and Place Signed
- 1F: Authorized Representative Signature
STEP 2 – Driver information should be provided in the following fields:
- 2A: Full Name (First, Middle, Last)
- 2B: Date of Birth
- 2C: WA Driver License Number
- 2D: Applicable Checkbox
- 2E: Employer, Prospective Employer, or Volunteer Organization Name
- 2F: Driver’s Signature
- 2G: Date Signed
How To Submit The Completed Form To GoodHire
Find the Candidate’s name listed on your GoodHire Background Check dashboard and click the Action link. The Action link will open the Candidate Consent page where you’ll find the section for Washington Abstract of Driving Record Release of Interest. Follow the instructions on the screen to upload the electronic copy to GoodHire. When the form is successfully uploaded, click Complete.
